The Position Job Title: Registered Nurse (RN) - Case Manager Job
Specialty: Case Management Job Duration: 13 weeks Shift: Monday to
Friday, 08:00-16:30 Guaranteed Hours: Full-time, 40 hours per week
Experience: 2-3 years of case management experience preferred
License: Licensed Registered Nurse (RN) or Licensed Practical Nurse
(LPN) Certifications: Relevant nursing certifications as required
Must-Have: Licensed in the respective state; proficiency in using
EPIC Electronic Medical Records (EMR) Job Description: - Provide
specialized case management services in an acute rehabilitation
setting - Collaborate with a multidisciplinary team to develop and
implement patient care plans - Manage patient caseloads effectively
to ensure quality of care and compliance with facility standards -
Facilitate communication between patients, families, and healthcare
providers - Coordinate discharge planning and post-discharge care
to ensure continuity of patient care - Maintain meticulous
documentation and records in the EMR system - Adhere to business
