A company is looking for a Field RN Case Manager in the Toledo
Region, OH.
Key Responsibilities
Manage and coordinate patient care in the field
Conduct assessments and develop care plans for patients
Collaborate with healthcare teams to ensure quality care
delivery
Required Qualifications, Training, and Education
RN Associate's Degree or higher
Current unrestricted state RN licensure
Case Management Certification (CCM) preferred
At least 2+ years of clinical experience
Home health experience preferred
