A company is looking for a CA Licensed Newborn/Pediatric Nurse
Care Manager.
Key Responsibilities
Deliver coordinated, patient-centered virtual Care Management by
telephone and video that improves members' health outcomes
Create impactful care plans with members and the care team to
achieve desired goals
Help members navigate complex medical conditions, treatment
pathways, and the healthcare system
Required Qualifications, Training, and Education
Current CA nursing license
Experience in care management or pediatric nursing
Ability to work remotely and manage time effectively
Strong understanding of healthcare systems and resources
Experience in providing compassionate, longitudinal follow-up
care
